[Linerva-announce] Linerva rebooted by malfunctioning KVM

There was an unscheduled linerva reboot at 10:56pm-11:09pm today. While
I was performing maintenance work on an unrelated SIPB server, the KVM
device (which is not keyboard controllable) malfunctioned and switched
from that server to linerva (which was three inputs away). This caused
a Ctrl+Alt+Del sequence to be delivered to linerva, which triggered a
shutdown and reboot.
We have taken two steps to ensure that this accident is not
repeated: we have unplugged it entirely from the KVM, and we have
disabled the Ctrl+Alt+Del trigger in linerva's inittab. We are aware
that linerva has had a pathetic uptime record over the last several
months. However, we are confident that we know the causes of all past
linerva outages and taken action to prevent these problems from
reoccurring.
