[Dspace-general] Release form

I've searched the DSpace list serv archive, I haven't seen too many 
references to release forms that allow institutions to collect & 
maintain documents.  I think the type of form is similar to copyright or 
author release agreements, but maybe this isn't what these types of 
forms are called.

My institution has some collections we want to place Online (e.g. Senior 
theses, or a collection of images from a faculty member).  To do so, we 
need a release statement from each author/creator that will allow the 
library to retain the right to manage these collections.  I've looked 
Online and have some samples agreements, but I'm looking for more.  Does 
your institution have or know of some similar agreements?

I'm new to this list serv, as well as to DSpace, and would appreciate
