Thanks for the further info.
So if I understand you correctly, the note created via UserDecisionNote,
will be seen as an attachment to the task.
The standard workflow terminates after sending the note back to the
creator. So using the UserDecisionNote, I will have to loop back after the
task completes, so that the initial user has visibility of it as an
attachment (as opposed to receiving it as an email).
I have made the changes to the xml file (as per SAP help on
UserDecisionNote) , renamed it and uploaded it back, with a priority
'high'. Cleared caches, but dont see any changes to how it was previously.
Does the box need restarting in order to recognise these changes or should
it be immediate, or is there a step I have missed.

Exactly the same as when using SOFM.CREATE - using an attachment. No
further work should be necessary in the rest of your WF (assuming you are
using SOFM.CREATE to add comments as an attachment to the WF).
