[OWW-SC] Survey Implementation

Hi SC,

Vincent and I have been looking into implementing the oww user  
survey.  We have looked into two survey-hosting options (wufoo.com  
and freeonlinesurvey.com), but both charge a fee for doing more than  
a small number of surveys (smaller than what we need).  We were  
wondering if

1.) There are other hosting options you know of that we can use for  
free.
2.) If we should pay for an account with oww funds.
3.) If we should put some effort into implementing the survey  
ourselves (via php, a mediawiki extension, rails, etc.)
