What a great document! I have been on top of our inventory for the most part, but have to justify safety/artistic necessity by item, every time.
Just replaced our entire sound system after 18 years-the speakers that came down [from their inaccessible location] turned out to have been several years beyond their useful life. Not in good shape at all.
I have maintained what I call my wish list that shows what I believe are the priorities, but I like your format better.

Here's a link to a Google Doc I'm using to outline equipment management here. Of course it doesn't guarantee that anything will actually be done, but it does keep us in the que for resources. The real key is to update it every year and keep sending it in with the new information.

In the meantime, we generally have some scraps of budget laying around at the end of the fiscal year, and we generally put it towards gear of one sort or another.
