SRM Display Agents - Multiple Decision Sets

Any ideas as to what triggers this ? Seems to be coming in somewhere when getting process information.

Process Controlled Workflow using BRF - looking at whole cart for dollars
Not sure when this started happening or how many users are being affected. We only know of 2 at the moment.
In production, in the display agents grid, the second level approver does not see a line of who approved the first level when it comes to him .
After he approves, the agents grid then shows "Multiple Decision Sets" and the next 2 lines are indented and will show the 1st level approver, and the second level approver.
It is like they are grouped under the header Multiple Decision Sets.
It seems to behaving as if it was a parallel approval.
We have tried to re-create in our test system with the same user and cart set-up, but it seems to behave as expected.
When the level 2 approver gets it, he sees the line of who approved it at level 1.
After he approves it, the grid shows the 2 lines as expected with approver (no indention and not grouped under any test called 'Multiple Decision Sets') .
This is the expected behavior.
Any ideas of what to check in production that may be causing this? Any activations or config, we may have overlooked.
Why would this start happening?
