Duplicate Work Items

For your requirement , you want to send a single work item (create and
change or either of the case).As per my suggestion, use SAP_WAPI FM in user
exit SAVE_DOCUMENT and add the condition
if Sy-tcode= VA or VA.
use wapi FM/select query  to check weather your WF is in process/under
approval.
write your logic.
