Error workitem already completed in UWL

Hi,
We have a custom workflow executing in UWL.If the approver1  doesn't approve within 90 hours it goes to review manager.
In one case the approver and review manager are  same users. After the deadline is reached the review manager is getting an error
Cannot execute as work item status is completed(It's trying to execute prev work item)But in Log its under second level approval.
Is there some transaction for a buffer refresh etc.I have tried deleting cookies swu_buf etc.
