Approval Preview Not appearing properly for SRM Workflow

Hi All
         This is Arghadip. I am trying to implement the Workflow WS14500015(Item Level Based Approval) for Shopping Cart. The Approval Preview is not appearing properly. It is not showing the Agent who approved the Shopping Cart. Can anyone help me in this regard. This one is quite urgent and any help is highly appreciated.
