Workflow jumping back to previous version

Hi,
We are facing a very peculiar problem with our workflow versions in the test
system.
All of a sudden, some of the workflows revert to a previous version even
though no TR has been moved to this effect.
Then we have to move a TR reactivating the current version.

Has anyone faced such a problem?
What could be the reason and solution?
