Two workflow instances started

I meant are your events delivered via the event queue? All events in error end up in the queue, but is your linkage set up to use the queue?
One situation I know about is if your queue parameters are too high then you can get jobs overlapping. This will cause duplicates.

see the lovely "Waiting because of e", in the Queue browser, via
SWEQBROWSER.
