Two workflow instances started

 We have the same issue here.   In our case, the event is raised at
exactly the same time.  Container is the same, but one processes, while
the other usually fails in the event queue with "Waiting because of e"
an extremely descriptive error message.
 
Date and time stamps are identical.    The event is raised by a custom
ABAP, and only gets executed once.
 
It is very rare that we get duplicates but it does happen.   Real hit
and miss.
 
Looking forward to see if anyone else has heard of this or found a
solution
