It happened sometimes at the site I am when the users executed the workitem via MAPI (which we have now phased out as it is very unstable and not supported) As soon as it is executed in the SAP Business Workplace - no problem.

We had this (and still have this) in some of our workflows. It doesn't seem to influence the run though. (although I'm never sure)
In our case it has to do with SUBMIT or COMMIT commands directly in the WF Object. So e.g. directly coded in a method of an object you use in the WF.
We don't use submit's nor commit's anymore directly in the object code but use e.g. a function to submit a program. (which uses a seperate process as far as I know) Maybe you could look up the method that this certain outcome calls and check the coding on submit or commit code.

To fix the problem for now, re-create (not copy it) the user decision step,
copy the steps in the branches/paths from the old one into the branches of
the new and then delete the old one.
I have found that these strange things happen with user decision steps when
I add, change or delete new selections.

We have a generic decision workitem using a bespoke method to provide a
generic pop-up on the DECISION object.
When control is returned to the workitem after the user has made a
selection, we are getting the error "Transaction brackets of the workflow
has been damaged".
This error is consistently on one of outcomes, and the other outcomes do
not cause a problem at all.
Does anyone have any idea what may be causing this and how we might fix it
?
We are on a 4.7 system here.
