Workflow '63011967' work item 63016296: No successor defined for event?

[Rick Sample]

No terminating step. No deadlines at all on this task. 
The same user is logged as "Complete by hand". 
I thought the only way "Complete by Hand" happened was by WF Admins? 
i.e. got to the wf log, take over task, and complete step manually.

Does this step have a terminating event? If the step does not have a
"latest end" defined, could it be that someone is manually completing
the work item?  What is the user ID by the "Completed by hand" message?

Normal log:
Dialog work item created     
Work item reserved           
Execution started            
Work item processing complete

Hi Rick,

Have you changed the workflow recently, without generating a new
version?
That kind of error is normally happened if you has changed & activated
the workflow, but the last workflow item is on going.
How if you try to restart that workflow manually by triggering event
SWUE?
