Workflow '63011967' work item 63016296: No successor defined for event?

[Rick Sample]

In the WF log I am seeing all these errors occurring after a "completed
by hand".
Can someone refresh me memory and tell me what these means?
 
Log view when I get the error message:
Dialog work item created       
->Completed by hand              
Work item reserved             
Work item processing complete  

 
Normal log:
Dialog work item created     
Work item reserved           
Execution started            
Work item processing complete

Hi Rick,

Have you changed the workflow recently, without generating a new
version?
That kind of error is normally happened if you has changed & activated
the workflow, but the last workflow item is on going.
How if you try to restart that workflow manually by triggering event
SWUE?
