Lotus Form Issue : Form not being displayed

Hello All,
 I am facing a problem in the lotus form in my work flow ,
 the form has been created and binded to a task ( FORM . PROCESS ), but the 
form is not visible in the lotus , and it has been shown as a "Work Item" 
rather than a form.
 i am in 4.6 c, i am sending the R3 form name , type correctly... could 
anyone give me some wayof solution.
