MSS - Personnel change requests

Hi All,
 
We're in the process of setting up the workflow process for PCR's in MSS and have come across a problem. Whenever we hit the 'Submit' button (for example in the 'Change employee group and subgroup' PCR) we get the following message :
 
'Name not known; Check your entries'
 
Has anyone else had this problem ?
