Workitem does not execute.

All,
I am using a subworkflow to create an attachment using the object
Selfitem-note_create. The workitem should
execute automatically prompting the user to create an attachment but it
instead goes to my workflow inbox. When I
execute from the workflow inbox it executes successfully and the workflow
completes. This object selfitem-note_create
is an synchronous object and is running in the foreground. Can anyone tell
me how I can trouble shoot this problem ?
