Work Item Not reaching Inbox.

Hi Work Flowers :-)
 
I can see work item in my outbox,But i can find the same in users outbox.
 
There is background Function Module to pick agent which is working perfectly find.
 
What could be the posible reasond for not reaching inbox.
 
This WF is in our QA system we r testing.In dev it is working fine.
 
Is there any thing like Inbox Initialization.
 
all the WF behaving same way as this work item not reaching inbox,being in out box.
