Workitem error - workflow administrator is not notified

Karen,
 
I assume you are calling a custom program for your update function?
 
We are calling a standard SAP transaction, FBV0, which must be
triggering the 3 terminating events, CHANGED, DELETED, POSTED, that
this task is expecting. Before I investigate further custom code, I'd
like to better understand why the workitem which is clearly in error,
is not being routed to the workflow administrator automatically.
