Workitem error - workflow administrator is not notified

Yes, I did attend BC601 & BC610. SWU3 was covered but the override in
the workflow header data was not covered. I figured that out back on
the job after the classes. By the way, I did not figure out what the
workflow template "responsible person" does (PFTC_CHG -> Additional
data -> Person responsible -> Create). Do you know?
 
In the case I mention below, the workflow administrator set via SWU3
has not been overridden by the workflow header data. Therefore, my
expectation is that our workflow administrator would receive a notice
when the error occurred, but she did not. In fact the same error
occurred a half dozen times before someone contacted her to research
what was happening.
 
The workitems in error were executing a custom asynchronous method
which was attempting to post an FI document via FBV0. The errors were
caused because the person executing the step lacked the necessary
authorizations for posting FI documents. I find the documentation
concerning exception handling and asynchronous methods to be somewhat
confusing, but I'm assuming that since the workitem was in error, the
lack of exception handling in this asynchronous method is not the issue.
 
So, I'm still at a loss as to why the item did not appear in the
workflow administrator's inbox. Any ideas?
