We had a similar problem.  If I recall correctly we had to add R/3
authorizations for faxing and printing to the profile for SAP MAPI to work
correctly.  I've sent a request to our security team to find out the exact
authorizations, but they seem to be on vacation.  If they get back with me I
can forward the info to you.
