Workflow definitions disappear

Perhaps this is totally wrong, but still worth a check:
 
please try accessing these "missing" workflows via "Workflow Builder" (SWDS)
instead of "Task Maintenance" (PFTC).
I believe there may be some issues because of the versioning of workflows.

Just thought I'd post an update:
Two people from OSS have been in our system, seen the effects, but
(understandably) the cause remains elusive without being able to
reproduce the problem.
All developers are on alert to check and - if it happens - try to
remember what they did, and try to reproduce it. If anyone out there
experiences these disappearing workflow descriptions and can reproduce
it, I'd appreciate any input. We still have the OSS call open, but as
Murphy would have it, this is not happening at present.
