WF for PR release not in users inbox

Please ignore the first cry for help.
 
Correction: the workitem is not going to the user through SWI5 and there is
no agent in SWI2.
 
We set-up a training client with 20 training ID's, Train01-Train20.
Workflow is triggering but not routing the PR workitem to Train01's inbox,
the other 19 are working perfectly.
*You cannot see the workitem for train01 through SWI5.
*There is no agent for the workitem  (SWI2).
In the PR, the release strategy says it's going to Train01.
Org enviornment was refresh through Train01's WF inbox.
Authorization appear to be correct.
It appears there's something wrong with the user ID, authorizations, or ???
 
Help me pleeez, any ideas?
