Error WL 411

I'm getting the following intermittant error on a workflow which up to now
has had no problems:
 
Workflow nnnnnnn work item nnnnn: No successor defined for event '0000'
 
In three out of the four examples I have it the error occurred on the last
step which has completed successfully.
 
I checked OSS for wl 411 and in note 37115 it states that the error has been
corrected from release 3.0D and in note 60930 that it has been correct as of
rel 3.0F/3.1G.
 
Any ideas?
