Work Centers in 4.6C

We are in the process of upgrading from 4.0B to 4.6C.  We have finally been
able to fix one of our issues with workflow.  However, we still seem to be
having problems with work centers.  We set these up when several people are
in the same group and can approve a document (i.e. Grants, Purchasing,
Accounts Payable, etc.).  In our Production environment, everything looks as
expected.  However, in our QA environment for 4.6C, the work centers are not
displayed correctly.  I am attaching a Word document that shows the screen
from the workflow audit trail.  If anyone can help us determine what we may
be missing, please do so.  It looks like it may have something to do with
the task; but, I'm not quite sure if that is the case.
