Refresh of organizational environment

Yes, this would happen if you have changed responsibilities somewhere in the
workflow and some (but not all) users have changed in the group of possible
agents.

Has someone experienced that it is necessary to refresh the
organizational environment in order to get all workitems in some
users inbox (but not necessary for others) even though the organization
chart was not changed? (This has happened a few times.)
If yes, what could possibly cause this? (version 4.6c)
