Workitem text is diferent as description

Hi All,
 
I have a problem in my production workflow system.
Sometimes when user displays work item in their inbox,
the work item text is diferent as work item
descripition. Work item text is correct but
description isn't( the descripition showed is
reference for another workitem ).
 
Anybody could help me ?
