Instance linkage problems

Please disregard item 2 below.  I have two events with similar names and
I picked the wrong one!  SWUE event creation does in fact recognize
instance linkage receivers.  My workflows are no longer stranded.
 
Item 1 is still a problem.

I've two instance linkage problems I am hoping someone can help with.
I'm working in release 4.6c, AIX, DB2.
 
1) I have a wait step in wf definition.  Every time I make a change to
the workflow definition, the wait step cannot be terminated for only the
first instance of the workflow after the change.  The second, third and
all subsequent instances of the workflow have no problem terminating the
wait step, until I make another change to the definition.  Has anyone
seen this bizarre phenomenon where the terminating event is not received
by the wait step just the first time?  The event log says that no
receivers were found. I have verified that the appropriate entry is in
the instance linkage table.
 
2) Related to #1, I have a workflow instance that is stranded.  It's not
in error, just in process.  The event has happened and I cannot create
it again via the application.  I want to manually raise the event, but
the "Create event" utility apparently does not recognize instance
linkages as legitimate receivers.  Does anyone know how to manually
terminate the wait step and pass event parameters back to the workflow
that is waiting?  I am not using the event queue.
