I got an excellent template from an ASUG presentation given by Gretchen
Roberts.  It is a Word document and I highly recommend it.  If you
cannot find the template on ASUG or do not have access please let me
know and I can forward it to you.
 
Also, another reply had mentioned using Visio.  We had originally used
this tool but are now switching to a tool called P-Tech.  This tool has
a lot more functionality than Visio and is allowing us to design,
develop and document the relationships in our workflows.  This would
include things such as parameters, objects, tables, etc.
