[Habitat Exec] Habitat for Humanity Spring Break Form

Hey all,

Cynthia, thanks for putting this together, it looks good. One comment,
maybe include a little more information on the financial side of things ie
they have to pay for their flight, we cover the rest or something like that.

Anyone else have any comments before we send this out?
