Guidelines and Commitment Acknowledgement:
I understand that:

* In addition to the program fee mentioned above, my group owes the full affiliate contribution of $1300 to the host affiliate.

* Once my team has paid the program fee of $30/person to HFHI-Collegiate Challenge, we are committed to pay the affiliate contribution for the number of confirmed participants, even if all do not participate.

* All participants must be 16 years of age or older by the first day of the trip.

* My group is responsible for arranging and paying for all transportation.

* If this form and the $30/person program fee has not arrived to the Collegiate Challenge office by 2/1/2013, this trip will be cancelled.
