[mit-habitat-exec] Tax Exempt Form

Just a heads-up, you usually do need both pages of ST-2 if our record is 
not on file.  You also generally have to explain that the second page of 
ST2 expired in 2004 and the state issued the letter on the first page 
rather than a new form.

It's crazy, but it usually works.
