issues with SPNEGO security contexts

A while ago, we had a potential design discussion where we were looking
at ways to collapse out the outer context--trying to detect if an inner
mechanism returns a glue context and if so returning that instead rather
than double layering.

I don't know if that is the right answer, but at least in a white board
meeting we did work through some of the requirements there.
