Ticket 5895 - race conditions in memory ccaches

Ticket 5895 discusses race conditions in krb5_mcc_initialize() and
krb5_mcc_destroy().  I have reviewed the patch provided and have
attached a revised version for review.

I will be happy to commit this change after it has been reviewed
by Consortium staff members.
