[krbdev.mit.edu #7409] rework documentation tree layout

Once the restructuredtext documentation becomes the only form of
documentation (#7408), the doc tree can be collapsed so that the
rst_source lives in just doc/, and similar transformations.
Where make targets are run and content generated (build versus source
tree) can also be reconsidered at that time.
