[krbdev.mit.edu #3930] CCAPI server must be able to distinguish context handles from other server instances

If a client obtains a context from a server and the server is then
restarted, the new server instance must be able to distinguish between
contexts it has issued and those that were from a previous instance.
This will be implemented as a platform specific server ID which will be
incorporated into all ccapi messages.
