Error "24" returned from INIT_CONTEXT under load

We are getting an error 24 (integer value of 24) returned from the 
init_context routine when I call it very frequently (sequentially - just 1 at 
a time).

Given all the error numbers are big -ve number, what is this error and what 
might be causing it.
