[Editors] Generic permission form

Hi!

I am looking for samples of permission forms used for web sites at MIT 
--preferably something all-encompassing that includes use of photos, 
posting of research, etc. If anyone has something like this, I'd be most 
appreciative of your forwarding a copy.
