[StarCluster] crash report

Hi Wayne,

Thanks for submitting the crash report. I'm able to reproduce your
crash if I run the 'terminate' command and then press Ctrl-D. I filed
a bug for this and already committed a fix for it:

https://github.com/jtriley/StarCluster/issues/80

This will be included in the next 0.93.2 release.

Concerning your failed cluster please try using the 'restart' command
instead of terminating and starting a cluster. This will avoid wasting
instance hours by rebooting and reconfiguring the nodes rather than
terminating and creating new instances.

Lastly, can you elaborate on 'failed start does not exit cleanly'. I'd
like to get a better understanding of the issue behind this.
